Exosomes For Hair Loss

EV’s are product isolated from human bone marrow MSCs that contains growth factors and extracellular vesicles including exosomes. They are one of the most exciting therapy tools in regenerative medicine today. They have been used by clinicians in wound care, burns and aesthetic applications including post laser/post micro needling and hair loss therapy.

Exosome Treatment

The Board Certified Plastic Surgeons at Illume Cosmetic Surgery have incorporated EV treatment as part of their armamentarium of tools for the treatment of genetic hair loss.

EV’s are cell-derived vesicles that are present in many eukaryotic fluids, including blood, urine, and cultured medium of cell cultures. They are packaged inside cellular endosomes and secreted from the cell, and they are responsible for direct cell to cell communication.

Extracellular vesicles provide a natural and comprehensive activation of the body’s healing processes by systematically activating the complex array of cell signals required to restore and maintain tissue health.

As the technology and discovery of new methods of treating patients has evolved, scientific evidence shows that the optimal way to provide true regenerative therapy is to directly provide the signals as its own therapy and avoid potential issues with delivering and keeping transplanted cells alive.

Although not specifically FDA approved for hair loss, extracellular vesicles have shown promising clinical results for treating hair loss from physicians around the world.

Case Study – Male Pattern Baldness

Patient: 46 year old Male

Treatment:

  • 2ml of xoGlo extracellular vesicles, diluted to 10ml with saline
  • Treatment given approximately 0.5cm apart
  • Results shown are at 2 weeks post procedure

Results:

At 2 weeks, patient has already shown new hair growth.*

Male Pattern Baldness

Patient: 43 year old male with receding frontal hair line as well as thinning of the crown area typical of male pattern baldness

Treatment:

  • Applied 5cc xoGlo EV’s diluted 1:1 treatment 1cm apart evenly across the frontal hair line as well as the crown.

Post-Treatment:

  • Performed microneedling on patient’s head at a depth of 1.0 to create additional trauma in the treatment areas.

Results:
Noticeable results first observed 3 days out from initial treatment on the crown of thread where hair was still present but thin while noticeable results were observed at 2 weeks in the frontal area where hair was fully missing. Significant filling and coverage was observed on the crown area at 17 days from initial treatment.*

Female Androgenic Hair Loss

Patient: 31 year old female with long standing history of diffuse genetic hair loss (androgenetic alopecia).

Treatment:

  • Single treatment with exosomes and micro needling
  • 2ml xoGlo EV’s diluted with 8ml sterile saline
  • Used 31 gauge needle with 10 1ml insulin syringe
  • Treatment 0.1cc per treated site approximately 1cm apart

At one month, patient has already shown new hair growth.*

What are exosomes?

Exosomes are extracellular vesicles that are produced by mesenchymal stem cells which are involved in cell communication. Other cells react to the messaging signals from the exosomes and will change their behavior accordingly. Scientists are now able to isolate and deliver these beneficial signalers directly rather than the stem cells themselves.

Exosomes are isolated from donated human amniotic mesenchymal stem cells and purified using proprietary processing. The unique benefit of these exosomes is that they are stem cell-derived and bring a wide array of growth factors. Because they’re so small and nimble, exosomes can remain hidden in the bloodstream, and carry multiple doses of proteins across barriers that cells are not able to cross.

What is exosome therapy for hair loss?

By injecting exosomes and growth factors, it has been clinically shown to help regenerate and regrow hair as a treatment for hair loss in both men and women in the earlier stages of hair loss.

How long does it take to see results from exosome therapy for hair loss?

Exosome therapy may begin promoting natural hair growth in as little as two to three months. New hair growth can continue over the course of a year, with the most noticeable results usually being seen six months after treatment has been administered.

How long do I have to wait before another session of exosome therapy for hair loss?

We typically recommend waiting 6 to 8 months after the last treatment to re-evaluate and before potentially performing another treatment of exosome injections if it is necessary.

How are the exosomes processed, and are they safe to use?

Our exosomes are an extracellular signal product isolated from human bone marrow MSCs that contains growth factors and extracellular vesicles including exosomes.

Extracellular vesicles are 30-150 nm in size and are involved in direct cell-to-cell communication.

Growth factors and EVs, including exosomes, are known to stimulate bioactivity and direct cellular communication.

The exosomes are isolated from living human bone marrow mesenchymal stem/stromal cells (MSCs) donors. The extracellular vesicles are purified using proprietary cGMP processing and are sterile filtered and tested per USP<71> sterility testing.

How many exosomes are in each vial?

For hair loss, we utilize 400 plus billion exosomes per vial. We have found that a 400 billion exosomes product is more than sufficient to regenerate hair growth.

How are exosome treatments different from PRP (Platelet Rich Plasma) treatments?

PRP or platelet rich plasma begins with centrifuging the patient’s blood in order to concentrate the growth factors and platelets naturally present in the body. The drawback of using PRP is it does not provide any stem cells or exosomes because as we age our bodies contain less and less stem cells in our blood. In addition to age being a factor, the patient’s health is also very critical to the quality of PRP harvested.

Before PRP was developed, stem cells would come from fat collected during liposuction. However, that process was painful and more expensive with the patient having to undergo two procedures. Again, the patient’s age and overall health came into play with the quantity and quality of the stem cells available.

We use exosomes which are sourced from amniotic mesenchymal stem cells. These exosomes are cryofrozen and packaged in a sterile vial ready for use.

How long do exosomes last in your body?

Exosomes are present 6 to 8 months after the final treatment.

Can the patient take NSAIDS?

No. It is ideal for the patient to stop taking NSAIDS for 72 hours prior to the injection and for 4 days after. NSAIDS will NOT destroy the injected cells but they can potentially lessen the effects.

Are there any contraindications for exosome injections?

There are no contraindications for exosome injections.

What preparation do I need for the day of the procedure?

Patients are advised to:

  • Drink a lot of fluid one day prior to the procedure
  • Have breakfast the morning of their procedure
  • Avoid Aspirin, Naproxen, Ibuprofen and other NSAIDs 3 days prior to the procedure and 3 weeks after the procedure.

Are there any restrictions after the procedure?

The only restriction is to avoid aspirin, naproxen, ibuprofen, and other NSAIDs for 3 weeks following the procedure. Following the procedure, you may go home and shower. Be aware the treated area may be tender. Many patients return to work or their daily routine the same day.

Can I combine exosome therapy with other hair loss modalities?

Absolutely! We recommend patients use Propecia, Finasteride or Minoxidil in conjunction with exosome therapy for hair loss.

Do exosomes cause cancer or cause cancer to grow?

Currently there are no large direct studies that can show exosomes or stem cells do not cause cancer, but there are studies where stem cells have been utilized to treat cancer, and the conclusion from these studies is that since the stem cells work to suppress cancer cells, it is assumed that they will neither help proliferate cancer cells nor cause the formation of cancer cells.

Can exosomes be used for Alopecia Areata?

Yes. Clinical and anecdotal results have shown tremendous efficacy for when exosome therapy is utilized.
